Experts agree ownership did not drive outcomes in COVID-19 long term care home outbreaksScience, not ideology, must drive our thinking on sector reform-REVERA
Mississauga, Ontario. March 22, 2021. We all want answers for why, during the worst pandemic in living memory, Canada’s long term care homes experienced large outbreaks with devastating outcomes. The pandemic impactedthe entire long termcare sector—non-profit and publicly-owned homes aswell as privately-owned onesexperienced severe outbreaks. Understanding the factors that contributed to the likelihood oftheseoutbreaks requires rigorous analysis by experts with relevant academic training in statistical analysis who have professional credentials in the medical and public health fields. Fortunately, this kind of analysis has now been conducted and we have a growing body of work thatcaninformthe discussion on how to bring aboutmeaningful and necessary improvements to the sector.It is tempting to look for simple solutions. Some observers and advocates have taken ideological positions or conducted rudimentary analysis with the view to link the ownership model of long term care homesto outcomes. Over the last year, expert analysis hasconcluded repeatedlythat the extent of community spread is the key factor in assessing the cause of an outbreak,and that the age and design of abuilding is the most statistically significant factor in determining the severity of an outbreak. Ownership model was not a significant factor.Let’s review some of thesestudies.On February 19, the [Ontario LTC] Commission heard from Michael Hillmer, Assistant Deputy Minister, Capacity Planning and Analytics, Ministry of Health. You can read the full transcript of Mr. Hillmer’s testimonyon the Commission’s website. He and his team have closely examined data from both Wave 1 and Wave 2 of the pandemic in long term care homes in Ontario, and have also looked at the data from the lens of ownership model –privately-owned homes, not-for-profit homes and municipally-owned homes. He indicates that his analysis found no association between ownership and outcomes. Specifically, in terms of for-profit versus not-for-profit status and the impact overall through Wave 1 and 2, Mr. Hillmer concluded that ownership model was not a factor “in outbreak, speed or mortality.”Rather, Hillmer’s data review shows that community spread is “the single biggest determinant of whether you have an outbreak.” The review also indicatesthat homes with an older design standard were more likely to experience alarger outbreak, but designwas not a significant factor in whether the home went into outbreak to begin with.The Canadian Institute for Health Information (CIHI) completed an analysis,using complex models and looking at data on a large number of variables, at the request of the Ontario Long-Term Care COVID-19 Commission. At the LTC Commission hearing on March 3, in response to questioning about the causes of outbreaks, Dr. John Hirdes, on behalf of CIHI, explained that “the distinction between the for-profitand not-for-profit homes was not significant.” (See p.63,Canadian Institute for Health Information transcript at LTC Commission, March 3, 2021)On March 15, anopinion piecepublished in the Toronto Starby Dr. Bob Bell and Dr. Walter Wodchis spoke about what Ontario has learned from the COVID-19 pandemic. Both men have extensive expertise. Dr. Bellis a former Deputy Minister of Health for Ontario, as well as a retired surgeon and hospital CEO, and the chair of the expert advisory panel for Revera’s report The Perfect Storm: The COVID-19 Experience for Revera and the Long Term Care Sector. Dr. Walter Wodchis is a professor of public health at the University of Toronto’s Dalla Lana School of Public Health.These two experts conductedsimple statistical analysis which determinesthatward roomswhere there are 3 or 4 people to a room and a shared bathroom -not ownershipmodel -is associated with mortality in long term care homes with COVID-19 outbreaks. Here is an excerpt from the op ed, which is accessible to subscribers of the Toronto Star:To evaluate what factor is primarily associated with COVID-19 mortality, we determined whether each For Profit and Not for Profit homein the province housed three or more residents in a room. Complete information was available for 510 homes. We excluded municipally owned homes which (unlike FP and NFP homes) receive extra funding from municipal taxes.We documented how many residents dieddue to COVID-19 in each home to March 1, 2021. Thankfully, vaccination has dramatically reduced COVID deaths following this date. The number of COVID deaths was divided by the number of residents to determine a mortality ratio for each home.A standard statistical test called “multiple regression” was done to figure out whether the ownership model (FP versus NFP) or the presence of three or more people in a room was associated with a higher mortality ratio.The answer is clear. FP ownership was not linked to higher mortality (for statisticians, this factor had a p-value of 0.43). However, the presence of three or four people in a room was strongly associated with higher mortality (p=0.002). This routine statistical analysis showed that the Star conclusion about for-profit care increasing mortality was wrong.We are pleased to seetheToronto Starpublishedthis op ed which demonstrates that the narrative their editorial team, along with others in the news and social media,has been espousing since early in the pandemicis factually incorrect. The prime minister also freed up a spot on the Likud list for National Union MK Ofir Sofer in order to coax Smotrich, who had been hesitant to merge with Otzma Yehudit, which until recently had been considered beyond the pale. The premier also reportedly agreed to back a number of pro-settlement policies and reserve senior posts for Religious Zionism MKs in the next government in exchange for the parties agreeing to unite.The DMFI statement also noted that the the Ra’am party, “based on Islamist ideology, may determine who leads the country’s next government.”The group did not specify why it made a point of noting the Islamist ideology of the Arab Israeli party.Mansour Abbas’s Ra’am is indeed slated to play the role of kingmaker, after campaigning on the notion that it is “not in the pocket of either the left or right-wing” and will be willing to cooperate with either bloc. Ra’am is on course to receive four seats in the next Knesset. With the pro-Netanyahu bloc including Yamina currently at 59 seats and the anti-Netanyahu bloc at 57 seats, Abbas will either play a major role in deciding the makeup of the next coalition or send the country to a dizzying fifth election in just over two years.Joining DMFI in condemning Religious Zionism’s success was T’ruah, a rabbinic human rights organization in North America.“We particularly condemn the inclusion of Kahanist and anti-LGBTQ parties in the new Knesset. SEOUL, South Korea (AP) — North Korea on Thursday test-fired its first ballistic missiles since US President Joe Biden took office, as it expands its military capabilities and increases pressure on Washington while nuclear negotiations remain stalled.Japanese Prime Minister Yoshihide Suga said the launches threaten “peace and safety in Japan and the region,” and that Tokyo will closely coordinate with Washington and Seoul on the North’s testing activities.South Korean Foreign Minister Chung Eui-yong, after meeting his Russian counterpart in Seoul, expressed “deep concern” over the launches and urged the North to uphold its commitments for peace. Russian Foreign Minister Sergey Lavrov called for a swift resumption of dialogue to resolve the standoff with North Korea.South Korea’s Joint Chiefs of Staff said the two short-range missiles were fired at 7:06 a.m. and 7:25 a.m. from an area on the North’s eastern coast and flew 450 kilometers (279 miles) on an apogee of 60 kilometers (37 miles) before landing in the sea.“This activity highlights the threat that North Korea’s illicit weapons program poses to its neighbors and the international community,” said US Indo-Pacific Command spokesperson Capt. Mike Kafka.The launches came a day after US and South Korean officials said the North fired short-range weapons presumed to be cruise missiles into its western sea over the weekend.North Korea has a history of testing new US administrations with missile launches and other provocations aimed at forcing the Americans back to the negotiating table.Still, Thursday’s launches were a measured provocation compared to the nuclear and intercontinental missile tests in 2017 that inspired war fears before the North shifted toward diplomacy with the Trump administration in 2018.Analysts say that the North would gradually dial up its weapons displays to increase its bargaining power as it angles to get back into stalled talks aimed at leveraging nuclear weapons for badly needed economic benefits.North Korea has so far ignored the Biden administration’s efforts to reach out, saying it won’t engage in meaningful talks unless Washington abandons its “hostile” policies.It’s unclear how the Biden administration would respond before it completes its policy review on North Korea in the coming weeks.The missile launches followed a trip by Secretary of State Antony Blinken and Defense Secretary Lloyd Austin to Japan and South Korea last week as Washington pushes to restore its alliances in Asia in the face of challenges from North Korea and China.During the trip, Blinken sternly criticized North Korea’s nuclear program and human rights record and pressed China to use its “tremendous influence” to convince the North to denuclearize.North Korean state media said Tuesday that leader Kim Jong Un reaffirmed his country’s traditional alliance with China while exchanging messages with Chinese President Xi Jinping in an apparent response to Biden’s efforts to coordinate action on North Korea with his allies.The negotiations over the North’s nuclear program faltered after the collapse of Kim’s second summit with then-US President Donald Trump in February 2019, when the Americans rejected North Korean demands for major sanctions relief in exchange for a partial surrender of their nuclear capabilities.Since Trump’s first meeting with Kim in 2018, the North has not conducted nuclear or long-range missile tests, although analysts believe they have pressed ahead with their programs on both.The North has continued short- and medium range missile testing during its suspension of nuclear and long-range tests, expanding its ability to strike targets in South Korea and Japan, including US bases there.Kim Dong-yub, an analyst from South Korea’s Institute for Far Eastern Studies, said the flight data released by South Korea’s military suggests the North possibly tested a new solid-fuel system modeled after Russia’s 9K720 Iskander mobile ballistic missiles.The low-flying missiles, which analysts see as potentially nuclear capable, are designed to be maneuverable so they have a better chance at evading missile defense systems.Kim said it was also possible that the North tested its new multiple rocket launchers or another solid-fuel ballistic system that outwardly resembles the US MGM-140 Army Tactical Missile System.The North had conducted at least 16 launches of the new short-range weapons since they were first revealed in 2019.Trump had been accused of giving North Korea room to advance its weaponry by repeatedly dismissing its short-range missile tests despite the threat they posed to South Korea and Japan.If Biden takes a different approach by imposing additional sanctions over short-range missile launches, the North may use it as an excuse for more provocative tests, including those involving submarine-launched ballistic missile systems, said Cheong Seong-Chang, an analyst at South Korea’s Sejong Institute.Kim Jong Un’s powerful sister last week berated the United States over its latest round of combined military exercises with South Korea that ended earlier this month, describing the drills as an invasion rehearsal and warned Washington to “refrain from causing a stink” if it wants to “sleep in peace” for the next four years.South Korea’s Defense Ministry said the North’s short-range tests on Sunday were its first missile firings since April 2020. Biden played down those launches, telling reporters, “There’s no new wrinkle in what they did.”
