Deaths Raise Safety Concerns Around Gene Therapy-Prospects grow cloudy for technology just when it appeared to have turned a corner-By-Oct. 1, 2021 5:30 am ET

Safety scares, including deaths of several young study subjects, have set back efforts to bring more gene therapies to market, clouding one of biotech’s most promising technologies and hottest sectors.Gene therapy, which replaces defective genes, promises to deliver one-time treatment for inherited diseases. Two treatments are approved for use, while dozens are in development.In September, however, a young boy with a rare neuromuscular disease died after receiving a gene therapy in a clinical trial run by Japan’s Astellas Pharma Inc., ALPMY -0.06% and regulators halted a BioMarin Pharmaceutical Inc. BMRN -0.82% gene-therapy study after discovering cancer in mice that received the treatment.What do you make of recent gene-therapy safety scares? A few years ago, “the skies were blue, and now that is not the case.”BioMarin’s share price fell 8.4% on the day after announcing the study pause, shedding $1.3 billion of its market value. Astellas shares fell 2% in Tokyo on the day following its disclosure of the patient death.Across nearly 30 publicly traded gene-therapy developers, share prices have declined by an average of 25.6% so far in 2021, compared with a 6.9% rise in the Nasdaq Biotechnology Index and a 14.7% increase in the S&P 500.Some gene-therapy researchers, however, say the spate of negative headlines is a symptom of the field’s success, which has led to an increase in gene-therapy treatments being tested in patients.“One of the reasons you’re hearing about all these [setbacks] is that the scale of these trials is going up exponentially because the therapies are all working so well,” says Nicole Paulk, assistant professor of biochemistry and biophysics at the University of California, San Francisco. “With that increase in numbers, there will be increases in growing pains.”Gene therapy aims to treat diseases such as hemophilia and certain forms of muscular dystrophy by providing working copies of the genes that are defective or missing in people with inherited diseases. Typically, the new gene is carried inside a harmless virus (injected into the patient), which acts as a vessel to insert healthy DNA into cells.Since 2015, 30 gene-therapy companies have gone public in initial-public offerings, raising $4.8 billion, according to data from Piper Sandler. More than 30 gene-therapy developers and suppliers have been acquired in deals valued at up to $43.5 billion since 2016, according to Piper.Most gene therapies first pass through the liver before they can get to their intended cell targets, and it has long been known that high doses can cause liver damage. Researchers at the National Institutes of Health and elsewhere have in recent years also found a potential long-term risk for cancer in mice and dogs treated with gene therapies.The risk of cancer resulting from gene therapies like the kind that BioMarin is developing has so far been seen only in animal studies, which aren’t always good proxies for how drugs will act in humans, said Henry Fuchs, president of world-wide research and development at BioMarin.“Liver cancer as a result of gene therapy has never been observed in a human, and so at this point it remains a theoretical concern, not an observed risk,” Dr. Fuchs said.BioMarin faced an even bigger setback last year when the FDA unexpectedly refused to approve BioMarin’s gene therapy for the bleeding disorder hemophilia, saying it wanted more detailed, long-term data before allowing the therapy on the market. “That surprise was a year-long setback,” Dr. Fuchs said.BioMarin expects in the coming months to have two years of study data that the FDA requested and to file for approval in 2022, Dr. Fuchs said. The company has said available data indicate the therapy is effective for at least five years, but won’t be a lifetime cure.Scientists have been working for decades to make gene therapy a reality, tantalized by the allure of curing inherited diseases such as cystic fibrosis by providing patients with functional copies of their defective genes.More than 20 years ago, a single gene-therapy study death all but halted the research in its tracks. In 1999, a mostly healthy 18-year-old named Jesse Gelsinger died of multi-organ failure after receiving an experimental gene therapy for a rare liver disease.In the aftermath, some large companies and academic researchers abandoned the research, and venture-capital funding dried up. The research that continued mostly ended in failure.Over the past decade, however, the field made a comeback. The FDA approved the first gene-therapy product in 2017, clearing the use of Luxturna to treat an inherited type of vision-loss affecting up to 3,000 people in the U.S. In 2019, the regulator cleared Zolgensma to treat a muscle-wasting disease that affects as many as 500 babies born in the U.S. each year.The lucrative market for rare-disease treatments, in which companies can charge high prices for small patient populations, helped trigger a wave of deal-making for gene-therapy makers. In 2018, Novartis AG had acquired Zolgensma-maker AveXis for $8.7 billion. The following year, Roche Holding AG agreed to buy Spark Therapeutics, developer of Luxturna, for $4.8 billion.Also in 2019, Astellas paid $3 billion to acquire San Francisco-based Audentes Therapeutics Inc., developer of gene therapy for X-Linked myotubular myopathy, an often-fatal rare disease in which muscle weakness prevents normal bone development in babies, leading to difficulties breathing and swallowing.Last year, researchers reported preliminary clinical trial data showing that many of the young boys treated with the Astellas therapy, code-named AT132, had rapid improvements in motor functions such as crawling and walking and their ability to breathe without ventilatorsYet since the summer of 2020, four boys have died in the Astellas study of children younger than five years after developing liver dysfunction in the weeks after treatment.Astellas researchers are still investigating the latest patient death in their gene-therapy study, but think that certain X-Linked myotubular myopathy patients are particularly susceptible to liver damage because of their underlying disease and that there is no evidence that an immune reaction to the virus delivering the healthy gene is causing the problem, said Nathan Bachtell, head of Astellas Gene Therapies.The company hopes to find a way to safely restart the clinical trial, possibly by excluding enrollment of patients with a high risk of liver dysfunction, Dr. Bachtell said.Write to Joseph Walker at joseph.walker@wsj.com-Appeared in the October 2, 2021, print edition as 'Safety Concerns Increase Around Gene Therapy.'

 The first thing that happened was the arrival of a gene-editing tool known as CRISPR in 2012.Many of you have probably heard about CRISPR so I'll just say briefly that CRISPR is a tool that allows researchers to edit genes very precisely, easily and quickly.It does this by harnessing a mechanism that already existed in bacteria.Basically, there's a protein that acts like a scissors and cuts the DNA,and there's an RNA molecule that directs the scissors to any point on the genome you want.The result is basically a word processor for genes.You can take an entire gene out, put one in,or even edit just a single letter within a gene.And you can do it in nearly any species.OK, remember how I said that gene drives originally had two problems? The first was that it was hard to engineer a mosquito to be malaria-resistant.That's basically gone now, thanks to CRISPR.But the other problem was logistical.How do you get your trait to spread? This is where it gets clever. A couple years ago, a biologist at Harvard named Kevin Esvelt wondered what would happen if you made it so that CRISPR inserted not only your new gene but also the machinery that does the cutting and pasting.In other words, what if CRISPR also copied and pasted itself. You'd end up with a perpetual motion machine for gene editing.And that's exactly what happened.This CRISPR gene drive that Esvelt created not only guarantees that a trait will get passed on,but if it's used in the germline cells,it will automatically copy and paste your new gene into both chromosomes of every single individual.It's like a global search and replace,or in science terms, it makes a heterozygous trait homozygous.So, what does this mean? For one thing, it means we have a very powerful, but also somewhat alarming new tool.Up until now, the fact that gene drives didn't work very well was actually kind of a relief. Normally when we mess around with an organism's genes,we make that thing less evolutionarily fit. So biologists can make all the mutant fruit flies they want without worrying about it. If some escape, natural selection just takes care of them.What's remarkable and powerful and frightening about gene drives is that that will no longer be true.Assuming that your trait does not have a big evolutionary handicap,like a mosquito that can't fly, the CRISPR-based gene drive will spread the change relentlessly until it is in every single individual in the population.Now, it isn't easy to make a gene drive that works that well, but James and Esvelt think that we can.The good news is that this opens the door to some remarkable things.If you put an anti-malarial gene drive in just 1 percent of Anopheles mosquitoes, the species that transmits malaria, researchers estimate that it would spread to the entire population in a year. So in a year, you could virtually eliminate malaria.In practice, we're still a few years out from being able to do that,but still, a 1,000 children a day die of malaria.In a year, that number could be almost zero.The same goes for dengue fever, chikungunya, yellow feve.And it gets better..Say you want to get rid of an invasive species, like get Asian carp out of the Great Lakes.All you have to do is release a gene drive-that makes the fish produce only male offspring.In a few generations, there'll be no females left, no more carp.In theory, this means we could restore hundreds of native speciesThat have been pushed to the brink.OK, that's the good new-this is the bad news.Gene drives are so effectivethat even an accidental release could change an entire species,and often very quickly. Anthony James took good precautions.He bred his mosquitos in a bio-containment and he also used a species that's not native to the US so that even if some did escape,they'd just die off, there'd be nothing for them to mate with.But it's also true that if a dozen Asian carp with the all-male gene drive accidentally got carried from the Great Lakes back to Asia, they could potentially wipe out the native Asian carp population.And that's not so unlikely, given how connected our world is.In fact, it's why we have an invasive species problem.And that's fish.Things like mosquitos and fruit flies,there's literally no way to contain them.They cross borders and oceans all the time.OK, the other piece of bad news is that a gene drive might not stay confined to what we call the target species.That's because of gene flow, which is a fancy way of saying that neighboring species sometimes interbreed. If that happens, it's possible a gene drive could cross over, like Asian carp could infect some other kind of carp.That's not so bad if your drive just promotes a trait, like eye color. In fact, there's a decent chance that we'll see a wave of very weird fruit flies in the near future.But it could be a disaster if your drive is deigned to eliminate the species entirely. The last worrisome thing is that the technology to do this, to genetically engineer an organism and include a gene drive is something that basically any lab in the world can do.An undergraduate can do it.A talented high schooler with some equipment can do it.Now, I'm guessing that this sounds terrifying.(Laughter)-Interestingly though, nearly every scientist I talk to seemed to think that gene drives were not actually that frightening or dangerous.Partly because they believe that scientists will be very cautious and responsible about using them.(Laughter)-So far, that's been true.But gene drives also have some actual limitations.So for one thing, they work only in sexually reproducing species.So thank goodness, they can't be used to engineer viruses or bacteria.Also, the trait spreads only with each successive generation.So changing or eliminating a population is practical only if that species has a fast reproductive cycle, like insects or maybe small vertebrates like mice or fish.In elephants or people, it would take centuries for a trait to spread widely enough to matter.Also, even with CRISPR, it's not that easy to engineer a truly devastating trait.Say you wanted to make a fruit fly that feeds on ordinary fruit instead of rotting fruit,with the aim of sabotaging American agriculture.First, you'd have to figure out which genes control what the fly wants to eat,which is already a very long and complicated project.Then you'd have to alter those genes to change the fly's behavior to whatever you'd want it to be, which is an even longer and more complicated project.And it might not even work,because the genes that control behavior are complex.So if you're a terrorist and have to choose between starting a grueling basic research program that will require years of meticulous lab work and still might not pan out, or just blowing stuff up? You'll probably choose the later.This is especially true because at least in theory, it should be pretty easy to build what's called a reversal drive.That's one that basically overwrites the change made by the first gene drive.So if you don't like the effects of a change, you can just release a second drive that will cancel it out, at least in theory. OK, so where does this leave us? We now have the ability to change entire species at will. Should we? Are we gods now? I'm not sure I'd say that.But I would say this: first, some very smart people are even now debating how to regulate gene drives.At the same time, some other very smart people are working hard to create safeguards, like gene drives that self-regulate or peter out after a few generations.That's great.But this technology still requires a conversation.And given the nature of gene drives,that conversation has to be global.What if Kenya wants to use a drive but Tanzania doesn't? Who decides whether to release a gene drive that can fly? I don't have the answer to that question. All we can do going forward, I think, is talk honestly about the risks and benefits and take responsibility for our choices.By that I mean, not just the choice to use a gene drive, but also the choice not to use one. Humans have a tendency to assume that the safest option is to preserve the status quo.But that's not always the case.Gene drives have risks, and those need to be discussed,but malaria exists now and kills 1,000 people a day.To combat it, we spray pesticides that do grave damage to other species including amphibians and birds.So when you hear about gene drives in the coming months,and trust me, you will be hearing about them, remember that. The scientist Richard Dawkins sparked controversy when he tweeted that, aside from the moral problems, eugenics would work “in practice.” While that remark is shocking, Dawkins is hardly alone in accepting the premise at the heart of eugenic science and population-control theory. Last year, a group of 11,000 scientists signed a statement urging population control to slow human exploitation of Earth’s fragile resources. With climate change finally a topic of urgent debate, some have argued that limiting population growth — if not eugenics — could be part of saving the planet.The idea that people should reproduce less to preserve our shared resources is nothing new. It is an old idea with a violent history. When reformers have encouraged — or forced — women to have fewer children in the name of population control, it has been the most vulnerable people and those most likely to be deemed undesirable or unfit who have paid the price. While advocates for reproductive rights and green activists alike may call for support for greater individual bodily autonomy, they should be careful not to reinforce the dangerous, even eugenicist, forces behind encouraging population control, like those hinted at in Dawkins’s tweet.though the United States has not enacted an official, nationwide population policy, debates on population control have surfaced over the years across the political spectrum — involving leaders from President Richard M. Nixon to Vice President Al Gore. However, the dark underbelly of population control — from eugenics in the early 20th-century United States to mass murder during the Holocaust to compulsory sterilization in India in the 1970s and China’s one-child policy — has undermined serious conversation about global family planning or strategies to combat climate change, poverty and overpopulation by addressing population growth.The English statistician Francis Galton, Charles Darwin’s cousin, coined the term eugenics in 1883 after studying the heritable qualities of human intelligence and ability. The eugenics movement gained momentum in early 20th-century America as, among other things, a way to explain genius and good character as well as criminality, bad social behavior and “feeblemindedness.” Eugenicists sought to improve the human population and its gene pool through encouraging “fit” individuals to procreate (positive eugenics) and discouraging or preventing the reproduction of the “unfit” (negative eugenics). This led to the forced sterilization of thousands of Americans and, in the case of Nazi Germany, the justification for murdering millions of people.The population movement of the 20th century has shied from that legacy and focused on how family planning could be used to minimize damage to the environment and human life. Although these efforts have bolstered the cause of racist eugenics, they also had an active role in advancing women’s rights. Biologist and Zero Population Growth (ZPG) founder Paul Ehrlich attempted to educate Americans about the importance of having smaller families for the sake of the environment and human quality of life. Beginning in the late 1960s, ZPG expanded its activism, arguing that overpopulation contributed to virtually all of the world’s modern ills: traffic, pollution, overcrowding and poverty, among others. Yet, not coincidentally, Ehrlich’s work initially focused on poor cities in the global south, such as the Indian state of Delhi, as the problem and the real cause for panic — rather than the consumption rates of wealthy, majority-white countries with much higher populations and rates of consumption.The link between population control and the darker uses of eugenics was never eliminated, which led to inhumane solutions across the globe. The problem? Many population-control advocates focused on coercive population campaigns that targeted the most powerless people.In the 1970s, China’s one-child policy mandated contraceptive devices, abortion and sterilization for women who became pregnant with a second child. (There were exceptions, such as if the woman’s first child was a girl or born with a disability.) Although this policy ended a few years ago, it has resulted in persistent low fertility rates, a “profoundly skewed sex ratio” and, some researchers say, significantly higher crime rates in China.In India, a policy of forced sterilization during the 1970s also took the goal of curbing the population to the extreme, depriving citizens of the freedom and right to engage in voluntary family planning, mainly targeting poor men and some women. Even into the ’60s and ’70s, many poor women of color were subjected to forced sterilizations in the United States, often while seeking another type of surgery or after childbirth. Like eugenics, these compulsory initiatives sought to limit the reproductive rights of great numbers of men and women for the “health” of the nation.Even in the Zero Population Growth movement, anti-immigrant, pro-eugenics figures such as John Tanton found a platform for using concerns about population to advance racism, bigotry and nativism, as well as an “us” vs. “them” mentality. Tanton and others from the population movement helped pave the way for the anti-immigrant, nationalist rhetoric of the Trump administration.Such concerns about population fueled government-mandated solutions that undermined voluntary individual choice. Although some of ZPG’s members did advocate compulsory population policy, others said that giving women greater choice — through promoting reproductive rights and challenging the country’s deep-seated pronatalism — was the best, most humane solution to population growth. They may provide a blueprint for those worried about overpopulation today.As the largest grass-roots population movement in the United States, coercive and racialized approaches to population are a troubling part of the history of ZPG. But many of its earliest and youngest members did not support coercive measures to control the population. In fact, many believed strongly in sexual liberation and advancing women’s reproductive rights as part of their environmentalist goals. Here, providing education and resources was key.To address population growth, many grass-roots activists took the education of ordinary Americans into their own hands, distributing literature and leaflets, fighting legal battles alongside frustrated women who sought sterilization and being an active force in the abortion rights movement. They not only promoted birth control, but also held contests for free vasectomies and prescriptions for the pill. With catchy bumper sticker slogans such as “The Population Bomb Is Everyone’s Baby” and “Control Your Local Stork,” many members of ZPG emphasized the importance of population activism at local levels, highlighting the positive changes people could make for the world. Focusing on each person’s responsibility and action, in part, challenged the pessimistic, declensionist message of the modern environmentalist movement.For example, from 1971 to 1973, ZPG, along with the Association for Voluntary Sterilization (AVS) and the American Civil Liberties Union (ACLU), ran “Operation Lawsuit,” which provided resources and advice to men and women seeking voluntary permanent sterilization. ZPG members fought to give more Americans access to birth control, abortion, sterilization, pregnancy prevention and comprehensive sex education. In doing so, they maintained, parenthood would become more deliberate and voluntary and thus, fewer people would be born. But ZPG also advocated for wider changes to American cultural and social norms, which were largely pronatalist, pushing back against the idea that motherhood was the be-all and end-all of womanhood. Some labeled these norms “coercive pronatalism,” citing the problem of accidental teen pregnancy, attacks on abortion rights and the minimum-age requirements that hospitals placed on women who wanted to be sterilized.
