Health Canada’s approval of ABRYSVO helps to address the urgent need to minimize the spread of RSV infections – a disease with flu-like symptoms that can result in respiratory distress1,2,3, and lead to serious infection such as bronchitis or pneumonia, requiring hospital treatment4-ABRYSVO is the only RSV vaccine authorized in Canada for maternal immunization-Kirkland, QC, January 4, 2023 - Pfizer Canada ULC announces the Health Canada approval of ABRYSVOTM, its bivalent Respiratory Syncytial Virus (RSV) vaccine. The vaccine is indicated for the prevention of lower respiratory tract disease caused by RSV in individuals 60 years of age and older by active immunization, and the prevention of lower respiratory tract disease and severe lower respiratory tract disease caused by RSV in infants from birth through 6 months of age by active immunization of pregnant individuals.RSV is a common respiratory virus that causes mild disease with cold-like symptoms. Health Canada has approved the first vaccine for respiratory syncytial virus (RSV) for adults age 60 and over, but it may only be available in "limited" quantities for this fall's respiratory virus season.RSV is a common but highly contagious virus that appears like a common cold for most people. In more vulnerable populations, RSV can cause bronchiolitis — the inflammation of the small airways in the lung — or pneumonia, say experts. Health Canada approved manufacturer GSK's new vaccine called Arexvy on Friday. In an email to CBC News, the federal health body confirmed that it anticipates a limited supply of the vaccine will be available for the upcoming RSV season. This news comes after the U.S. Food and Drug Administration (FDA) gave their approval for the vaccine in May. "It will be a game changer in significantly preventing severe illness and death, especially amongst older Canadians," said Dr. Samir Sinha, the director of geriatrics at Sinai Health and University Health Network in Toronto. Doctors have been calling for an RSV vaccine for seniors because although the virus is common, people who are older are more likely to become severely ill and need hospitalization. First RSV vaccine approved in U.S. after decades of attempts-"This has been decades long in the making," Dr. Isaac Bogoch, an infectious disease specialist with the University Health Network in Toronto, said of the vaccine's approval. "It's a very tough virus, it can have negative health impacts. It [can] lead to hospitalization, intensive care unit stays, even death,"  Bogoch added. "And it's wonderful to have a vaccine that … appears to significantly reduce the risk of severe lower respiratory tract disease." Dr. Samir Sinha stands outside a hospital, wearing a white lab coat. (Ousama Farag/CBC)-RSV not well-tracked in Canada: experts-It's unclear how many people 60 and older in Canada are hospitalized or die from the illness, as experts say it's not properly tracked.According to Sinha, Canada isn't properly screening for RSV and the illness itself can be hard to detect — making the exact burden unclear. But he estimates that it can be similar to the rates of hospitalizations and death seen with influenza — if not worse as the virus can spread more rapidly and symptoms may take a few days to develop.In Canada, 12,200 people are hospitalized and 3,500 die from influenza annually. Vaccine offers more than 80% protection, says GSK-In a news release issued Friday, GSK said a randomized clinical trial showed the vaccine has an 82 per cent efficacy in preventing lower respiratory tract disease caused by RSV, compared to seniors who got a placebo.The company said it also showed a 94 per cent efficacy in preventing the illness in seniors with underlying medical conditions."I think this vaccine will go a long way based on the results that are available … the vaccine appeared to be safe and appeared to provide very significant protection," said Bogoch. THE DOSE-It took 60 years, but RSV vaccines are just around the corner — and could make a big difference-He added that if the infection spreads to the lower respiratory tract, it can make it hard for people to breathe so that they need "supplemental oxygen." This is when people typically end up in hospital. Sinha said RSV infections can also "trigger other problems in the body," like a bacterial or viral pneumonia. "So it's not necessarily that if you get RSV you're going to die of that infection, but what you might do is trigger other heart or other lung complications that then can actually cost you your life potentially," he said. National guidance for the shot expected in 2024.RSV season in Canada usually starts in the late fall and lasts until spring. Health Canada said the vaccine is a single dose injection, but it's unclear whether people will need to get the vaccine every year or a booster. St. Joseph's Healthcare Hamilton infectious disease physician Dr. Zain Chagla said available data suggests that people who get the vaccine will be protected for up to two years. But beyond that, he said further research is needed. "Hopefully this [vaccine] gets in before RSV season, but if an older adult accesses the vaccine afterwards, they still could get a couple years benefit," he said. In an email to CBC News, Health Canada said that guidance from the National Advisory Committee on Immunization (NACI) on the use of the vaccine is expected next year. Drug to help prevent serious RSV illness in babies approved by Health Canada-It said that based on consultations with Canadian experts, NACI has "first prioritized (and is currently developing) advice for RSV products to protect infants before developing advice for older adults."It added that it will be up to individual provinces and territories to decide if they will work Arexvy into their existing RSV programs — and if so, it will work with them individually. Experts like Sinha say they also hope the Canadian government considers covering the cost of the vaccine as that can often be a "barrier to access," especially in more vulnerable populations. Sinha said that as soon as the vaccine is made available, he'll be recommending that all his patients get the shot. What about kids? Last winter was an especially bad season of RSV in the country, specifically for young kids. In addition to seniors, infants are at a higher risk of getting very sick with RSV. There is no RSV vaccine for children, but there are two kinds of antibody injections that can be given to high-risk babies to help prevent serious illness.  One of them, palivizumab, has often been given to babies who were born prematurely – but it needs to be injected about once a month during RSV season to stay effective. A new antibody drug – nirsevimab, also known by the brand name Beyfortus – was approved by Health Canada in April. Nirsevimab only requires one injection to protect babies during the RSV season. It's not yet known how widely it will be recommended for babies in Canada this fall.  On Thursday, the Centers for Disease Control and Prevention (CDC) in the U.S. recommended that babies born just before or during the RSV season, as well as babies who are less than eight months old before the season starts, should get the nirsevimab shot. The CDC also recommended that the shot should be given to some eight to 19-month-old babies who are at higher risk of getting seriously ill from the virus.

The Shin Bet security agency on Monday revealed attempts by Iranian intelligence services to spy on Israeli defense officials and gather information on civilians by using fake social media pages related to the ongoing war and the hostages held by terrorists in the Gaza Strip.According to the Shin Bet, Iranian operatives, using fake online profiles, tasked Israelis with photographing the residences of Israeli defense officials and other officials who appear frequently in the media and speak out publicly against Iran.Using fictitious social media pages and websites, the Iranian operatives also initiated gatherings near the homes of the families of the hostages kidnapped by Hamas and had bouquets of flowers and messages sent to their homes, the Shin Bet said.The agency said that the Iranian operatives had taken advantage of the protests in support of the hostages by having Israelis hang signs relating to the issue that had been written by them, as well as asking them to photograph the demonstrators.The pages also asked Israelis to fill out surveys, which would require victims to give out personal information. The Shin Bet said this was an attempt to collect further information on Israeli civilians, to later use them to carry out various tasks.The social media profiles — on Instagram, Telegram and TikTok — included pages posing as those of far-right activists and supporters of releasing the hostages, as well as a page named Kan+, impersonating the Kan public broadcaster.The operation revealed on Monday was run by the same Iranian network behind attempts uncovered last month to recruit Israelis over social media to carry out spying missions, including an assassination, in exchange for money.In that operation, individuals affiliated with Iranian security services contacted Israelis on various platforms and sparked an initial conversation using various deceptions. An elderly woman was killed and at least 17 people were injured, including at least seven children and teenagers, in a multipronged Palestinian car-ramming and stabbing terror attack in the central Israel city of Ra’anana early on Monday afternoon.During the attack, the perpetrators — two West Bank Palestinians working in Israel illegally — seized at least three vehicles and rammed pedestrians in several locations in the city, also stabbing one or more of their victims, according to police, medics, and eyewitnesses.Meir Hospital said a woman in her 70s, who arrived in critical condition, succumbed to her wounds. She was not immediately named.The two suspects were named as Ahmed Zidat, 25, and Mahmoud Zidad, 44, both residents of the southern West Bank town of Bani Naim, close to Hebron. According to the Shin Bet, both had been blacklisted for entering Israel illegally numerous times in the past.The attack occurred as schools were dismissing students for the day.According to reports, one of the terrorists stabbed a driver on the city’s Haharoshet Street before stealing the car and driving it into three people.The terrorist then got stuck and abandoned the car, before stealing a second vehicle and continuing on, driving down the central Ahuza Street and ramming into at least one other group of people, witnesses said.“It all happened in a second,” a passerby who witnessed the start of the attack told Army Radio.“We heard the noise of a crash, like a car bumping into another car,” Eden Arzi said. “We saw the driver go to stab a woman and she ran away, and [the driver] went and stabbed a 60-year-old man while there were a bunch of screams in the background, until he fell on the ground.”Another witness told Haaretz: “I saw someone stab three people next to the mall, steal a car and run people over with it.”Casualties from the attack were taken to Meir Hospital in Kfar Saba, Ichilov Hospital in Tel Aviv, Beilinson Hospital in Petah Tikva and the adjacent Schneider Children’s Medical Center.Magen David Adom said that the injured included at least two in serious condition — a man aged 34 and a 16-year-old boy — along with nine in moderate condition, and six who sustained light wounds.Seven children and teenagers were brought to Schneider in moderate condition, some of them having been transferred from Beilinson Hospital, where they were initially taken.Ichilov Hospital said two victims of the attack arrived at the hospital, both men in moderate condition and suffering from orthopedic injuries. The Israel Defense Forces on Monday began to withdraw its 36th Division from Gaza for an R&R and training period, leaving three other divisions fighting Hamas in the Strip.The withdrawal of the division comes as part of the IDF’s plans for a long war against Hamas, as well as a potential escalation in the fighting against the Lebanese Hezbollah terror group in northern Israel, while maintaining the troops’ competence.The division’s units will be given a short break, then will return for a training period, after which the IDF will decide if and where to redeploy them based to its latest assessments.Over the past 80 days, the 36th Division operated in Gaza City’s Zeitoun, Shati, Shejaiya, and Rimal neighborhoods, where the IDF dismantled all of Hamas’s battalions. UK scientists have begun developing vaccines as an insurance against a new pandemic caused by an unknown "Disease X".The work is being carried out at the government's high-security Porton Down laboratory complex in Wiltshire by a team of more than 200 scientists.They have drawn up a threat list of animal viruses that are capable of infecting humans and could in future spread rapidly around the world.Which of them will break through and trigger the next pandemic is unknown, which is why it's referred to only as "Disease X".Sky News was escorted around the site, which is run by the UK Health Security Agency, to see the work being done in high-containment labs.Risk of future pandemics 'rising globally' Professor Dame Jenny Harries, the head of the UK Health Security Agency (UKHSA), told Sky News: "What we're trying to do here is ensure that we prepare so that if we have a new Disease X, a new pathogen, we have done as much of that work in advance as possible."Hopefully we can prevent it [a pandemic]. As the world eases back to normalcy following three years of COVID-19, the scientific and public health community is already preparing for the next serious outbreak or pandemic. They just don’t know what the disease will be yet.Scientists have estimated that 1.67 million yet-to-be-discovered viruses exist in mammals and birds, and about half of them have the potential to spill to humans. As far back as 2018, the World Health Organization (WHO) gave this unknown future outbreak a placeholder name: Disease X. It represents the “knowledge that a serious international epidemic could be caused by a pathogen currently unknown to cause human disease,” the WHO explained. One year after that designation, COVID-19 was identified as the first in the mysterious category that scientists had warned about.Today, more infectious outbreaks seem inevitable.“It is not an exaggeration to say that there is potential of a Disease X event just around the corner,” says Pranab Chatterjee, researcher at the Department of International Health at Johns Hopkins Bloomberg School of Public Health in Baltimore. “The recent spate of H5N1 bird flu cases in Cambodia is just a case in point.”It will take creativity and vigilance to track and anticipate the next outbreak, says Peter Daszak, president of EcoHealth Alliance, a New York-based global environmental non-profit organization. “Nature is producing new viruses all the time … What we’re trying to say (with Disease X) is, let’s think creatively about designing vaccines and therapeutics and drugs that not only affect known agents but also can affect future and emerging pandemic pathogens.”The next Disease X is likely to be zoonotic — caused by pathogens that “spill over” from animals to humans — since that’s the case with almost 75 per cent of emerging infectious diseases. Ebola, HIV-AIDS, rabies and COVID-19 come immediately to mind.Data suggests that nearly all of the recent infectious diseases of global concern, including COVID-19, are caused by animal viruses that gained human transmission. Hence, zoonotic viruses continue to be the pathogens of interest as future diseases with pandemic potential. Even those that are already from a known source could evolve into something new and threatening, says Barney Graham, senior adviser for global health equity at Morehouse School of Medicine in Atlanta.1.67 million unknown viruses-One example is Rift Valley Fever, a virus listed by WHO as one of eight known diseases posing the greatest public risk of a health emergency. In its severe form, it can result in blindness, excessive bleeding, and brain swelling. It usually spreads to humans via mosquito bites. No human-to-human spread has been documented, but RVF could be the next Disease X if its causative virus changes its transmission mode to respiratory spread, similar to flu viruses.All of the 1.67 million unknown viruses belong to about 25 viral families; already, scientists have identified 120 viruses from these families that pose a potential risk to humans. Since it’s virtually impossible to prepare for each viral pandemic threat, scientists have devised a strategy to identify prototype (model) viruses from these families that can potentially cause the greatest risk to humans, and for which there are no known medical countermeasures.Graham says this approach aims to develop diagnostics, antivirals, and vaccines for those prototypic viruses and then work on ways to find transferable solutions for similar ones. That strategy has already borne fruit. The rapid development of vaccines for SARS-CoV-2, the virus that caused COVID-19, was due to several years of research on its earlier family predecessors, SARS-CoV1 and MERS-CoV.“With the resources and technologies we have now, I don’t think it’s too much to ask to be ready for 120 different viruses … so, I just think it’s a call to action to have much more investment in basic research, and preparedness and understanding of biology and understanding of the threats,” Graham says.Considering that three out of four new outbreaks originate in wildlife, scientists have a fair idea of global hot spots where future Disease X might originate.“And you can predict pretty clearly that the places where new emerging diseases are most likely to start are countries where there’s a lot of wildlife diversity … means tropical and subtropical countries,” Daszak says.Adding to the potential sources are rising global temperatures that could unleash zombie viruses held frozen for thousands of years in the permafrost.Populations don’t have to live in tropical zones or hot spots to risk exposure, since the main drivers of the next Disease X pandemic seem to be land usage change, deforestation, population growth, and activities such as wildlife trade, where “humans come in contact with ‘new to us’ pathogens,” explains Jarod Hanson, chief content officer of Program for Monitoring Emerging Diseases (ProMED) in Maryland. “By the time we recognize the occurrence of a Disease X in humans, spillover has already taken place.”Indeed, Chatterjee says surveillance may be “a key approach in our ability to detect a spillover event before it becomes too widespread.”That’s where event-based surveillance systems such as ProMED come into picture. An internet-based reporting network, it is a transparent and apolitical system that is supported by the non-government, non-profit International Society for Infectious Diseases and is open to all and can be accessed free of charge.Hanson says ProMED, unlike government organizations, can inform the public about the initial case first captured by informal and/or local reporting, whether it be via social media, local media or when that case first presents to a health-care worker. Reports are curated by experts and key pieces of missing information are added.Because of its event-based reporting, ProMED is critical to the global public health community, especially when we talk about stopping/containing the spread, mobilizing resources, and creating a shared understanding of where we are responding to an outbreak.“There is no one perfect surveillance system, nor will there ever be,” Hanson says. “We have to remain vigilant for both known and unknown threats using event, artificial intelligence, routine, and lab-based surveillance. ProMED is one piece of the disease-surveillance space occupying a very specific niche where we try to identify outbreaks before definitive results are available.”COVID-19 is a great example of ProMED’s strength in evaluating the significance of informal information.“But it wasn’t until a local media source captured it and ProMED disseminated it that it was identified as a real threat,” Hanson says. “In outbreaks with pandemic potential, any extra days we can move the response to the left on the timeline means lives and dollars saved.”The global response to the COVID pandemic has laid bare and amplified the health inequities people face within and between countries. Seven out of 10 people in high-income countries have received at least one dose of COVID-19 vaccine, compared to just three in 10 in low-income countries. Those differences create fertile ground for new variants to evolve.“All global threats began as regional problems. Even HIV was a regional problem until we missed it for two or three decades, and then it became a global problem,” Graham, of the Morehouse School of Medicine, says. “And it’s in the high-income countries’ best interest to help low-income countries have the facility and capacities to solve their own problems regionally before they become global problems.”And “global problems” doesn’t just mean human problems. Zoonotic diseases teach us the critical need for a strategy that will consider the interconnection of human, animal, and environmental health at the interface of emerging outbreaks. One such approach is called One Health, which, according to Chatterjee of Johns Hopkins, is key to “effectively identifying and responding to these threats as it provides a trans-sectoral and multidisciplinary approach to identifying and responding to public health threats.”One Health calls for enhanced global action in tackling the world’s complex health challenges, such as Disease X in conjunction with organizations that deal with food, agricultural, environment, human and animal health. Among the lessons of COVID-19 is that we can’t focus solely on one issue and ignore all others. A near-singular focus on one disease means paving the path for more outbreaks.Monkeypox, a limited outbreak of Sudan Ebola virus disease, the Marburg virus outbreak in Equatorial Guinea, and massive influenza and RSV outbreaks in late 2022 — as well as measles continuing to rear its head — are all examples of outbreaks that have occurred due to far less attention given to high-risk diseases as we continued to try to do massive COVID-19 surveillance, says Hanson of ProMED.For pandemic prevention and preparedness, Graham says, it’s about taking a long-term view.“You know, we act like we’re only going to be on the planet for another five years. And we need to be thinking longer term, we need to be thinking outside of an election cycle.
